If you are a new patient, please call our registration department to schedule an appointment. Please bring the following information to your appointment:

Medical information such as medical records, x-rays or MRIs, Co-payments if required by your insurance carrier. Payment is due at the time of your appointment. Insurance information such as the insurance company name, address and policy number. We will make a copy of your card upon your arrival. For work-related and automobile injuries, we will need your insurance company name, address, your claim or case number, employer name, attorney name and your date of injury. If your claim has been denied, please notify us. If you have been referred by another physician, we will need his/her name and address. Insurance plans often require a referral from a primary care provider. Please consult your insurance provider before making an appointment
