Please call our registration department to schedule an appointment. When scheduling your appointment please have your insurance information available at the time of your call. Please bring your insurance information, a photo ID, any previous x-rays, MRIs, and medical records for your scheduled appointment. For work-related and automobile injuries we will need your insurance company name, address, claim or case number, employer name, attorney name, and your date of injury. If another physician has referred you we will need his/her name, address and phone number. Insurance plans often require a referral from a primary care provider. Please consult your insurance provider before making an appointment. Co-payments, if required by your insurance carrier, are due at the time of your appointment.

 

Should you have questions regarding your treatment or medication, our receptionist will be happy to relay the information to the appropriate physician or department.

Canceling an Appointment:
Cancellations should be made at least 24 hours in advance of your scheduled appointment time. If you will not be able to keep your appointment, please contact our office as soon as possible to reschedule.

 

Emergencies
For urgent questions before or after office hours, please call (727) 347-1286 and our answering service will take your call and notify one of the physicians. If you have a medical emergency please call 911.
